Dear Parents,
If you have not filled out a new Health History for this school year, please fill it out and return it to the Health Office as soon as possible. These forms are very important as we need the contact information in case your child is sick or injured at school.
If you need a new one , please stop by the office and we would be happy to give you another one to fill out.
