Important message for the Parents of 5th Grade Students
Arizona state law requires that all incoming 6th grade students have proof of the
following vaccinations by the first day of school for the 2018-2019 School Year:
1. Tdap – 1 dose is required when 5 years has passed since the last Tetanus shot and
the student is at least 11 years old.
2. Meningococcal – 1 dose is required for all incoming 6th graders, 11 years and older.
If you have any questions about the vaccination requirements, please contact your
school health office or read attached Arizona School Immunization Requirements: K-
12.
Updated immunization records may be turned into your school health office or
emailed to the addresses below.
